CS Application Checklist

- Students may obtain general off-campus program information from the current online Cornerstone University Catalog – Academic Information section.
- Complete CU Off-Campus Study Program application from Eagles’ Nest (Academics tab – Registrar’s Office link) and obtain the following signatures:
- Larry Rice, Director of Global Opportunities. Obtain a list of course equivalencies and discuss program details.
- Paula Helbert, Student Financial Services.
- Academic Advisor (Print Program Evaluation from Web Advisor and bring it along for advisor meeting to discuss how classes will transfer to a major or minor at CU.)
- Registrar’s Office (Form may be left for the Registrar’s signature – Gail Duhon.)
- Students must: Be a junior or senior. Be enrolled at least six credits the semester before and after the off-campus program except for student teachers and social work practicum students. Must have declared a major or program. Participate in only one program. Be able to use the credits from the off-campus program in their academic program prior to degree completion.
- Director of Ministry Development will provide students with a list of course equivalencies for the program if available. Any further specific questions may be directed to the Assistant Registrar.
- Teacher education students need their student teaching site approved by the Teacher Education Division prior to registration for student teaching.
- Contact your CS campus representative and inform them of your interest in attending the program to receive any necessary information about on campus application procedure.
- Notify the head of your academic department of your application to the program to obtain approval to apply.

- Download application at link below and complete in Microsoft Word. Do not handwrite your answers.
- Save as a Microsoft Word document. (File name example: Doe, John-Fall10-College.doc)
- Print a hard copy for your application packet.
- Visit your academic advisor and CS campus representative for signatures on your application.
- Recommendation Forms
- Direct two references (preferably one professor and one work supervisor) to download the recommendation form at link below and complete in Microsoft Word.
- References will submit hard copies of the recommendation forms to your campus representative and email electronic copies to silvian@chicagosemester.org per directions on the recommendation form.
- Resume
- Review our Resume Resources page for guidelines to follow when putting together your resume and consult your career services office on campus for help.
- Save resume as a Microsoft Word document. (File name example: Doe, John-Fall10-Resume.doc)
- Print a hard copy for your application packet.
- Transcript
- Request an official transcript from the registrar to include with your application packet.
- Include official transcripts from all colleges attended.
- Follow-up with references to confirm they have submitted hard copies of the recommendation forms to your campus representative and emailed electronic copies to silvian@chicagosemester.org.
- Submit hard copies of the following items in your application packet to your campus representative:
- CS Application – with signatures
- Two Recommendation Forms
- Resume
- Official Transcript
- Application Fee – nonrefundable $40 check to Chicago Semester
- Photograph – 2x2” (passport photos preferred)
- After submitting your application packet email the following items to silvian@chicagosemester.org.

- Apply for financial aid as a CU student as soon as possible on the FAFSA.
- All financial aid will be administered through CU’s Student Financial Aid Office for CU approved off-campus programs.
- Payment in full for the semester should be made to Cornerstone University by January 1 for spring programs, May 1 for summer programs, and August 1 for fall programs. No payment plans are available as Cornerstone University must pay the program administrators at the beginning of the semester.
- Students may not use employee remission and Outstanding Scholar Awards.
- Register for off-campus program through Registrar’s Office on a course registration form. List the specific courses by name. If the courses are not known, list the name and location of the program. Once the specific classes are known, send course titles to the Assistant Registrar (Amy_R_Dauer@cornerstone.edu) via e-mail.
- Request an official transcript from off-campus program administrator upon completion of program to be sent to the Cornerstone University Registrar’s Office. This will be evaluated and added to the student’s Cornerstone University transcript.

Admittance to Program
Once you have submitted your application packet, and have been approved by your college, the campus representative will send your application packet to the Chicago Semester office. Applications will not be processed unless complete. The Chicago Semester staff will review the applications and you will be sent a letter regarding acceptance by May 1 for the fall semester and July 1 for the spring semester. Your college’s approval to apply to the program does not guarantee acceptance to the program.
Enrollment Deposit
Within 30 days of the date of your acceptance letter, you must submit a nonrefundable $225 enrollment deposit to reserve your place. The deposit will be applied to your public transit card for the semester and ticket fee for the fine art events. If you withdraw from the program after submitting the enrollment deposit, it cannot be refunded. If upon class registration you decide not to attend the fine art events, you will be refunded $125 once the semester begins. Payments may be made by check to Chicago Semester or cash.
